Notes

Beulah's death certificate records Arthur Shook as her father and Molly Davenport as her mother. The informant was Fred Zlomke, and he got about half of that information correct. As stated in the obituary, Arthur Shook was Beulah's brother. Her mother was Molly Furr (not Davenport). The only document linking Beulah to her father is Theodrick's obituary, which sites Mrs. Beulah Zlomke as a surviving daughter. Likewise, the only document linking Beulah to her mother is Molly's obituary, which also sites Mrs. Beulah Zlomke as a surviving daughter.
